Elementals with the Water Elemental Model but with barkskin and entangle effect on them, low damage but extremely dangerous disease.
Aren't elementals supposed to be created from one of the four elements?

Actually, DnD also has Para-elementals and Quasi-elementals.
These are elementals that are formed from the composite of other elements.
Para-elementals are the meeting of two main elementals:
- Ice Elementals dwell where Air meets Water.
- Ooze Elementals dwell where Earth meets Water.
- Magma Elementals dwell where Earth meets Fire.
- Smoke Elementals dwell where Fire meets Air.
The Quasi-Elementals are formed where one of the four elements meets with either the positive or negative plane:
- Steam Elementals dwell where Water meets Positive.
- Mineral Elementals dwell where Earth meets Positive.
- Radiance Elementals dwell where Fire meets Positive.
- Lightning Elementals dwell where Air meets Positive.
- Salt Elementals dwell where Water meets Negative.
- Dust Elementals dwell where Earth meets Negative.
- Ash Elementals dwell where Fire meets Negative.
- Vacuum Elementals dwell where Air meets Negative.
That being said, we are playing a fantasy RPG. Who says we need limit ourselves? A Rot elemental sounds nifty to me!
Don't the Shambling Mounds somewhat fill this niche already?
Edit: Anyhow. there's no saying you couldn't make up a few elemental planes within reason. I've seen rules for a plane of wood before, and you could make rot and growth quasi-elemental planes from that.
Now my turn to suggest something.
Nessian War Hound
Found in instances that might involve someone who has palnar slaves/allies.
Also found on some islands that have connections to Baator.
Essentially, it is a level 12 outsider Hellhound with the Size Doubled. A Hellhound on Steriods and additicted to the rape and mutilation of anything from the Prime Material *Lol.*
