The Avatar of Death spirit summoned through the Death domain incurs an attack of opportunity on each attack it does.
It should either get a creature weapon + creature weapon feat, or some other fitting ethereal weapon(scythe?) + feat.
You need to mention which deity too :) Each deity has a different death domain avatar.
