A DM is unavailable on IRC or IG at the time of occurence, so I am posting compiled evidence here. A green acid fog killed two members of our party after exiting the Clowns illusion quest, taken to bleeding before loading had completed. Screenshots were not taken immediately at the time of death with the position of the fog, but it was located exactly where the corpses are positioned in the given screenshots.

The two PCs who were killed are Dmitra Izbraxx and Gaarak.
I am the player of Dmitra Izbraxx. We didn't stand a chance, I was dead the second I loaded in.

I wanted to post and confirm this as a witness.
Also a witness, I took a shot of my combat log. I was making rolls vs acid damage before I even got the popup telling me I was in a Full PVP area.
