I'm not sure if I'm missing something here but I've been noticing that I get a "failure" when climbing even though I've passed the DC.

In all cases you rolled '1'. It is automatic failure.

This can be considered a rare, but sometimes probable mistake. Misses step, grapping a lose stone, what ever might be possible.

It is not a bug, it's an intended feature.
