Character: Drogor Splints

My axe was just pulled from my inventory and I'm not sure what happened. I'm fairly certain it wasn't DM intervention that did it.
About five minutes prior to it vanishing it had dropped out of my inventory because I put it away while my bag was full. I did not receive a prompt that my axe was lost, and the only reason I checked was because I saw it pop out of its quickbar slot.
So I checked and confirmed that it was in my inventory and moved along.
While standing in the square I then received the prompt "Lost Item: Chop Axe"
I think I may have had a duplicate in my inventory, but I don't know where the original would have been since it wasn't on the ground when I initially had dropped it, and when I left Starag's Rest to go elsewhere cleanup scripts decided that it was junk on the floor and should be gotten rid of.
Currently trying to contact a DM in game or in IRC to help, unless I'm just out of luck with this. :(
