There's a large expanse without access to a Doorkeeper. I understand why this might have at one time been intentional and warranted, but the lack of one is really stark right now.

Maybe you should look again, there is two doorkeepers. One in the 70s and one in the 60s.
I was trying to be reasonably vague, but I'm specifically talking about 64-70. There's a pretty substantive distance (in danger) from the nearest doorkeeper in these rings, and it would stand to reason that one could be added in 65 or 66.
