Onset of Decay, which deals AOE negative energy damage will only harm a single target if it does a killing blow. (So if I am surrounded by 6 low health wolves, only one that would die would be harmed, maybe two, this behaviour is quite erratic but can very easily be replicated since it happens extremely often)
It deals damage to undead targets while giving off a healing vfx. (so, instead of healing friendly or hostile undead such as Ghouls or Skeletons, I would deal damage instead)
The mixup between damage/heal on undead is fixed for next reset.
I don't think the rest is accurate. Be aware that the radius of this turn undead is a bit smaller than others.
I don't think this is radius dependant, the aoe damage really doesn't go off if it kills a single target, here's a video as an example.

Maybe compare the script for the undead turn "Acidic Fury" and swap out acid damage for negative?
They do the same thing, only difference is the type of damage.
I suspect Acidic fury doesn't heal undead. That being said, I am also Sure in Tala's ability to fix the issue :)
