There is a map on the ground of Sanctuary's prominent buildings showing what has been accomplished in the rebuilding. One of them is still labeled as the House of Heroes though. Also the Refinery does not have a description like the others do. It simply states 'there is nothing special about this object'.
I'm not so sure that the "House of Heroes" thing is a bug. That map is a miniature module of Sanctuary, as far as I can tell (At least that what makes sense to me). The Grotto is a new addition to Upper's scenery and the map has been long before it.
I find it sort of nice to have something like this, a sort of "before" the House of Heroes was destroyed.
