Displacement Faction Issues

I created a displacement mirror image of Tenilue in the Open Door. A "Dwarven Ringrunner" npc immediately ran up and started attacking the mirror image. It was only this one npc that attacked, the other npcs in the area did not attack. All of the npcs were blue to me and blue to Tenilue.

I do not think that I have been involved in any events or combat that could have resulted in faction issues for my own character. Tenilue's player said the same.

Probably some AI glitch. I saw it happen once with another spell that's probably using the same functionality, somewhat. Let me know if there are other such odd scenarios.
