I've had this before as well - but only on certain results. To be clear, I have successfully gotten items out of herbalism with the green item before. It is possible that only certain results are causing this?

In my experience so far, the tables tell you when you fail outright, but I have never made an herbalism check/roll, and the feedback suggests something is created but nothing is.
