Kobold's Warren

Took this quest today with three others. Everything went normally up until we came to the second to last room before the queen. Two dead PC's had perma'd at the entrance, and the kobolds in the room as well as the boss didn't spawn. Traps and treasure were still intact, but there was no way to finish the quest as of that point without DM aid.
