Issues with /c desc

The replace option does not seem to delete the original term, so it simply appends the "to" option behind it.

Ie trying to replace "knife" with "surgical scalpel" seems to result in it then reading "knifesurgical scalpel".

Also, the reset option does not appear to work at all currently.

The thing about the substitute has been a problem since the first implementation of this feature.
