Death System Bug

I just encountered the following error while attempting to respawn from a death that results from Kraken Tentacles aboard the ship from Mycopolis to Blue Mushroom:

EFUVS:ERROR: Failed to shipwreck. Please find a DM
[Death System] Failure while attempting to respawn. This is a bug.

The dialog sequence is as follows:

The figure gazes at you expressionlessly. "Your friends are all slain, and there is no mortal left to save you. However, your time in the realm of the living is not yet over if you do not wish it to be.

Do you wish to return to your life on the material plane? You will lose much of your worldly wealth.

Yes, return my corpse to life.

The name of the PC is Oloorin Graveltoes.

After a certain login the PC above ends up in the DM Discussion Area with the message:

Your party was SHIPWRECKED, but you were logged out at the time. Cheating suspected. Please wait for a DM.

I did log out and in after the script error above, but DM Caster teleported me ashore afterwards. Can the said PC be moved out of the OOC area? Thank you.

Same thing happened to me, but only stuck in the fugue!
