Containers within backpacks disappear upon dying

On two occasions, my character has been fugued and upon return a backpack has disappeared.  The PC backpack was there with all my stuff, just the containers within "the backpack" is what I am talking about.
 
On one occasion, there were error messages about all of the things within it dropping.  As for the occasion last night, I am not sure as my game crashed as I searched for a DM on IRC.  And damn the person who brought a cat to the Outer Gate transition that ganked me while I was waiting for my screen to show up - even with a decent MS and Hide.  And damn this company laptop for crashing the game if I try to IRC at the same time.
 
Anyways, the things inside the backpack were there, but the actual container is long gone.  I prefer the packs as it helps keep things in order but if they are going to dissappear upon death, then I would rather spend my precious 20-30 gold on healing than a temporary pack.
 
This is a bug as it never happened while in the Underdark.
Any ideas?

It happened in the UD. Just a rare occassion, most likely due to death and lag... I think.

Suite ... I am a rare occasion, twice even.
I suppose it is better than when Zebulon fugued and the server crashed and all of his things were long gone, ... on numerous occasions.
