Behemoth Spider Quest

When taking this quest and going inside the lair it seems pretty consistently that when entering the Grey Dwarves don't spawn but the spider can still be killed. After the spider is killed the quest does not update and the script of "Slipping into the pit" with the spider still fires; The result is a difficult to escape dungeon with no pay and no aid for the spider.

This is still happening, can't turn in the quest cause the journal never updates on it's death.
