No key on Kedrian quest

I recently did the quest Specialist Kedrian gives out.  The final boss usually gives a key to get into a certain area, but didn't.

The only thing that may have screwed this up I can think of is that a player died and logged before we killed the boss.  We got the "quest finish" ding but the usual key didn't show.
